Copy fields from one pdf to another
The basic layout for navigation and of some standard questions and buttons remained. It would be more than helpful to be able to simply paste them into the new qestionaire.

Did those pages reflow at all, requiring you to move the fields? At that point you really need a manual process with Acrobat Pro. I agree with Oded, iTextSharp should be able to do the job.
